Tips: How should I prepare for my online appointment?

Online appointments at the Dialog Therapy Centre take place via your Patient Portal account.

BEFORE YOUR APPOINTMENT:

1. Pay for your appointment within 60 minutes of booking it via the Patient Portal (Blik, Przelewy24, credit card); otherwise, the time you have selected will be cancelled.

2. Fill in your personal data (PESEL number, address) on your Patient Portal account.

3. If you have the following medical records, please prepare:

  • hospital treatment records
  • laboratory test results (e.g., CT, MRI)
  • certificates
  • a list of medications you are taking currently and in the past, regardless of the cause
  • information concerning illnesses, accidents, hospitalizations
  • any documents and information concerning your health condition

4. Before your appointment, make sure your equipment (camera, microphone, speakers) is working properly.

💻 On your computer, you will be redirected to a video window on your web browser without needing to install any software. Make sure your browser has permission to use your camera and microphone so you can talk to your specialist without any problems. Detailed tips for checking your equipment before the appointment can be found HERE.

📞 If you want to have your appointment by phone, you must download and log in to the “Google Meet” app beforehand.

You can download the Google Meet app for iOS HERE

You can download the Google Meet app for Android HERE

In order to test your equipment, go to the test visit window HERE  and check your equipment by following the directions provided HERE.

5. A few minutes before the appointment log in to your account on Patients Portal and click the “Join the Conversation” button next to your scheduled appointment.

TO ENSURE THAT YOUR ONLINE APPOINTMENTS TAKE PLACE SMOOTHLY:

1
Prepare the appropriate device and Internet connection that will allow you to participate in your appointment (your smartphone is enough – you can use it to arrange a meeting, pay for it, and participate in it).
2
Organize your time so that you are available for the entire duration of the consultation (at the time of your choice).
5
The specialist will wait for the patient ten minutes from the start of the appointment, after which the specialist will end the session, considering it an unsuccessful attempt to reach the patient. If you have any questions, please contact the Patient Service Department: 22 436 83 50.
6
The meeting should take place in appropriate conditions, i.e., a comfortable and quiet place.
7
Mental health is a very personal area of one’s life. For this reason, online appointments should take place without the presence of third parties (e.g., family members).
9
The patient and the doctor/psychotherapist will use video and audio equipment at every appointment (unless otherwise specified).
8
Your appointment will take place via an Internet-based video chat that encrypts connections.

At the Dialog Therapy Centre, patients schedule appointments for a specific time. The entire team of the Dialog Therapy Centre will make every effort to ensure your scheduled appointment takes place at the agreed time. Unfortunately, there are situations in which a patient’s condition prevents the scheduled completion of a given appointment. As a result, we request that Patients reserve additional time in the event that they have to wait due to an extended appointment.
Thank you for your understanding.

DURING THE APPOINTMENT:

  1.     Your doctor will identify you based on an identity document (ID card, passport), so please prepare it in advance.
  2.     During the visit, the doctor may issue an e-prescription and, if necessary, an electronic sick leave certificate (e-ZLA). Your doctor may also refer you for tests or to other specialists (e.g., psychotherapy).

Some certificates and referrals are valid only in a traditional paper version with a signature and a stamp. Such documents may be picked up in person at any clinic in Warsaw or sent to patients by post.

On the  Patient Portal of the NZOZ Dialog Therapy Centre, not only do online visits with our specialists take place, but you can also easily:

  •       edit your address information
  •       schedule/cancel another visit
  •       pay for your visit (Blik, credit card, przelewy24, PayPal)
  •       check your e-prescription code and medical recommendations
  •       order medical records and review your consultation history at the NZOZ Dialog Therapy Centre
  •   submit documents and test results to your patient record so that your specialist can review them

We encourage you to use the Patient Portal!
