Tips: How should I prepare for a phone appointment?

Phone appointments at the Dialog Therapy Centre.

Phone appointments can be held only on a Polish phone number. You can schedule an online appointment if you do not have a Polish phone number.

 

BEFORE YOUR APPOINTMENT:

1. Pay for your appointment within 60 minutes of booking it via the Patient Portal (Blik, Przelewy24, credit card); otherwise, the time you have booked will be cancelled.

2. Fill in your personal data (PESEL number, address) on your Patient Portal account

3. Your doctor will contact you using the phone number provided in your patient record/at the time of scheduling – check this number in the Patient Portal
ATTENTION! The doctor may call you from a private (unlisted) number. Please ensure that your device does not block incoming calls from such numbers.

4. If you have the following medical records,  please prepare:

  • hospital treatment records
  • laboratory test results (e.g., CT, MRI)
  • certificates
  • a list of medications you are taking currently and in the past, regardless of the cause
  • information concerning illnesses, accidents, hospitalizations
  • any documents and information concerning your health condition

TO ENSURE THAT YOUR PHONE APPOINTMENTS TAKE PLACE SMOOTHLY:

Organize your time so that you are available for the entire duration of the consultation (at the time of your choice).

Make sure you are available by phone 5 minutes before your visit.

The meeting should take place in appropriate conditions, i.e., a comfortable and quiet place.

Mental health is a very personal area of one’s life. For this reason, online appointments should take place without the presence of third parties (e.g., family members).

Phone appointments are conducted via an audio call; a video connection is not required. However, if the doctor believes that a patient’s condition requires a personal examination, he/she may refer the patient for an in-person appointment at a clinic.

At the Dialog Therapy Centre, patients schedule appointments for a specific time. The entire team of the Dialog Therapy Centre will make every effort to ensure your scheduled appointment will take place at the agreed time. Unfortunately, there are situations in which a patient’s condition prevents the scheduled completion of a given appointment. As a result, we request that Patients reserve additional time in the event that they have to wait due to an extended appointment.

 DURING THE APPOINTMENT:

  • Your doctor will identify you based on an identity document (ID card, passport), so please prepare it in advance.
  • During the visit, the doctor may issue an e-prescription and, if necessary, an electronic sick leave certificate (e-ZLA). Your doctor may also refer you for tests or to other specialists (e.g., psychotherapy).
  • Some certificates and referrals are valid only in a traditional paper version with a signature and a stamp. Such documents may be picked up in person at any clinic in Warsaw or sent to patients by post.

On the Patient Portal of the NZOZ Dialog Therapy Centre, not only do online visits with our specialists take place, but you can also easily:

  •       edit your address information
  •       schedule/cancel another visit
  •       pay for your visit (Blik, credit card, przelewy24, PayPal)
  •       check your e-prescription code and medical recommendations
  •       order medical records and review your consultation history at the NZOZ Dialog Therapy Centre
  •       Submit documents and test results to your patient record so that your specialist can review them
